3   LONG TITLE
4   General Description:
5This bill addresses medical records.
6   Highlighted Provisions:
7This bill:
8▸ allows a patient to request that the patient's medical record reflect that a pregnancy loss
9   under certain circumstances was not elective; and
10▸ requires a health care provider to include a note in a patient's medical record if requested.

19   Be it enacted by the Legislature of the state of Utah:
20Section 1. Section 26B-2-244 is enacted to read:
2126B-2-244 . Medical records.
22   (1) An individual may request that a health care provider include in the individual's medical
23record and health history that a procedure was not elective if the individual experienced
24a pregnancy loss due to:
25(a) a miscarriage;
26(b) a procedure to remove a deceased fetus;
27(c) an ectopic pregnancy;
28(d) a necessary procedure to avert the death of the woman;
29(e) a necessary procedure to avert a serious physical risk of substantial impairment of a

30major bodily function of the individual; or
31(f) termination of a pregnancy that is a result of rape or incest.
32   (2) A health care provider that receives a request described in Subsection (1) shall include a
33note in the individual's medical record.
34Section 2. Effective Date.
35   This bill takes effect on May 6, 2026.
